Challenge

OakLeaf Surgical Hospital faced the need to migrate to enterprise-hosted EHR and support services without disrupting surgical schedules, perioperative workflows, or revenue cycle processes.

Results

By partnering with CereCore, a comprehensive EHR modernization strategy was created, beginning with a high-stakes "READY" implementation. By replacing legacy customizations with standardized, best-practice workflows, the team successfully transitioned the facility to MEDITECH Expanse 2.2. This initiative provided a modernized clinical environment, integrated specialized anesthesia products, and established a repeatable implementation playbook that Surgery Partners can now use to rapidly integrate future acquisitions into their enterprise network.

About the Provider

OakLeaf Surgical Hospital, a Surgery Partners facility, is a premier surgical center focused on orthopedic, spine, and general surgery. As part of a larger network, OakLeaf prioritizes patient-centered care and operational efficiency. The transition to a modern EHR was a critical step in their mission to provide providers with the best possible tools for clinical excellence.

A Roadmap for EHR Modernization

The engagement began with a Current State and Readiness Assessment. CereCore’s team evaluated OakLeaf’s technical infrastructure, application usage, and clinical satisfaction to build a roadmap for the transition. This due diligence ensured that facility leadership had a clear understanding of the investment and the operational changes required to move from a customized 6.0 system to the latest Expanse offering.

Key Outcomes:

Standardized Workflow Transition: Successfully moved the facility from a highly customized and unsatisfactory legacy 6.0 system to a standards-based MEDITECH Expanse 2.2 environment.

Integrated Clinical Suite: Deployed the full MEDITECH web application suite along with an anesthesia software, creating a seamless digital workflow for surgical teams.

Evidence-Based Roadmap: Provided a comprehensive "Current State and Readiness Assessment" that outlined technical infrastructure and application usage requirements prior to implementation.

Improved Data Visibility: Resolved previous reporting functionality gaps by implementing MEDITECH’s advanced reporting tools, providing leadership with better clinical and operational insights.

On-Time Project Delivery: Managed a complex 14-month implementation timeline using a dedicated project manager and specialized consultants to ensure all project milestones were met.

Implementing Best Practices at Scale

The implementation team, consisting of a CereCore project manager and specialized consultants, focused on a "standards-based" approach. By removing unnecessary legacy customizations, the team was able to implement MEDITECH’s latest Expanse 2.2 platform with high fidelity to clinical best practices. This 14-month project also saw the successful deployment of an anesthesia product and the full web application suite, creating a truly integrated digital environment for surgeons and staff.

“The project went so well. I am so proud of my team here, my MEDITECH team, our staff, our physicians, and certainly, we could not have done it if we did not have the right partners. For the CIOs on a project like this, it takes a village, and we were never afraid.”

Post-Go-Live Optimization and Success

Following the successful go-live, the partnership shifted toward optimization. The collaborative approach between OakLeaf, Surgery Partners, and CereCore ensured that clinical integration and reporting functionality were not just "active," but optimized for the hospital's specific surgical volume. The result was a high-functioning EHR that improved the provider experience and provided leadership with the data needed for strategic decision-making.
